To calculate your monthly premium, find your age group to locate the correct rate. The rates shown for STD are per each $10 of weekly benefit and the rates shown for LTD are per each $100 of monthly coverage.

Age 31
Rate is $0.467
Annual Salary: $50,000
$50,000 / 520 x 60% = $57.69
$57.69 x 0.467 = $26.94 monthly cost

LTD Example:
Age 31
Rate is $0.121
Annual Salary: $50,000
$50,000 / 1200 = $41.67
0.121 x 41.67 = $5.04 monthly cost
