Q: When will my benefits terminate?
A: Benefits terminate the last day of the month in which you separate from the company. Information to continue benefits under COBRA can be found in the Exit Packet
Q: What happens to the money in my Health Savings Account (HSA)?
A: The HSA is an account in your name so any money within the account remains there. You can access the account through HSA Bank’s website (https://www.hsabank.com/hsabank/homepage) or by contacting their Client Assistance Center at (800) 357-6246.
Q: How do I rollover/close my 401k?
A: Your 401k will still be available to access in ADP (https://workforcenow.adp.com/workforcenow/login.html). Additionally, the forms to request a withdrawal/rollover can be found here: Withdraw-Rollover Funds after Employment. Completed forms can be sent to hr@benchmark.us.
Q: What happens to the money in my 401k if I don’t take any action?
A: If your account contains at least $5,000 on your separation date, the funds will remain in Benchmark’s plan until you request a withdraw/rollover. If your account has less than $5,000 it will be subject to a forced distribution and you will receive notification from the Third Party Administrator.
